(1) Not enough visitors. This is the biggest reason that most sites using Adsense do not make that money. You need a lot of visitors if you expect to make a significant amount of money from Adsense.
(2) Bad ad placement. If you stick your ad low on the side of the page where nobody can see it, how do you expect them to click it? A little ad optimization goes a long way.
